Guidelines for Team Member Emergency Fund

1. Authority for the disbursement of funds to team members is vested in the King’s Daughters Health Foundation Board of Directors and/or the appointed Team Member Emergency Fund committee.
2. Team members applying for assistance should be in full time or part time status, be in good standing and have completed one year of employment.
3. Donations to this fund will be accepted but may not be earmarked for specific individuals.
4. The King’s Daughters Health Foundation will maintain records of assistance granted.
5. Any full-time, or part-time team member (in good standing) or any team member on an approved LOA, is eligible to apply for the funds.
6. Team member may apply for up to $2,000 per applicant per 12 month period.
7. The application must be approved unanimously by the appointed Team Member Emergency Fund Committee before assistance is given.
8. No cash is given to the applicant. Checks are made payable to the vendor and will be made available for pick up from the designated administrator.
9. A copy of the bill (invoices, letters from landlords, etc.) must be attached and show the applicant’s name, amount owed, date and whom to make the check out to; a new bill must be provided with each payment.
10. No “luxuries” such as cell phone bills, cable bills, or other entertainment bills will be paid.
11. No checks will be paid to any check cashing institutions.
All information is treated in a confidential manner.
1) Stewart Health Foundation Scholarship (One Available)
$2,000 annually, renewable for up to four years, as long as scholarship criteria are met. Use is limited to tuition, room and board, books and lab fees. Scholarship payment of $1,000/semester will be mailed directly to the recipient's school of choice at the beginning of each semester.
Eligibility: Must be a resident of the following states and counties at the time of application and award (Children of full-time team members at King's Daughters are exempt from residency requirement)
- Kentucky- Boyd, Carter, Greenup or Lawrence
- Ohio- Lawrence
- West Virginia- Wayne
Other criteria:
- Must be a high school senior and plan to enroll in a healthcare career field at an education provider in KY, OH or WV
- Maintain at least a 2.5 GPA in high school based on the 4.0 system or successfully completed the GED
- Scored 15 or better on the ACT or 700 or better on SAT
2) Boyd County Medical Society Scholarship (Four Available)
These $1,500 scholarships are for the first school year's tuition, room and board, books and lab fees for any field of study and are non-renewable. The scholarship will be paid $750/semester.
Eligibility: must be a senior at one of the following schools
- Boyd Co. High School
- Paul G. Blazer High School
- Fairview High School
- Russell High School
- Greenup Co. High School
- Raceland High School
Other Criteria:
- Applicant cannot be a son or daughter of a current member of: Boyd Co. Medical Society, KD Health Foundation Board of Directors, UK KD Board of Directors
- Must maintain at least a 3.0 GPA in high school based on 4.0 system
- Scored 20 or better on ACT or 900 or better on SAT
3) Edna and Mildred Monk Memorial Nursing Scholarship (Five Available)
These $1,250 scholarships are for the first school year's tuition, room and board, books and lab fees for a student pursuing a career in the field of nursing. A scholarship installment of $625 will be mailed each semester to the school. Scholarship is for one year and is non-renewable. Applicant must be a direct child or grandchild of a UK King's Daughters Team Member and in good standing at the time of application and award. Applicant must be a high school senior..
Eligibility:
- Applicant must be planning to enroll at an educational provider in KY, OH or WV with the intent to pursue an education in the field of nursing.
- Maintain at least a 2.5 GPA in high school based on 4.0 system or successfully completed GED
- Scored 15 or better on ACT and scored 700 or better on SAT
Thank you for your interest in requesting a UK King’s Daughters sponsorship. In order to be considered, your organization must have a non-profit designation. Your W-9 must be attached. If your request is an event or has a deadline, it needs to be submitted at least 6 weeks prior to the date. Late submissions cannot be guaranteed review.
